STD Awareness: Is Chlamydia Bad?

chlamydiaPerhaps your sexual partner has informed you that they have been diagnosed with chlamydia, and you need to get tested, too. Maybe you’ve been notified by the health department that you might have been exposed to chlamydia. And it’s possible that you barely know what chlamydia even is, let alone how much you should be worried about it.

Chlamydia is one of the most common sexually transmitted diseases (STDs) out there, especially among young people. It can be spread by oral, vaginal, and anal sex, particularly when condoms or dental dams were not used correctly or at all. It is often a “silent” infection, meaning that most people with chlamydia don’t experience symptoms — you can’t assume you don’t have it because you feel fine, and you can’t assume your partner doesn’t have it because they look fine. If you’re sexually active, the best way to protect yourself is to know your partner’s STD status and to practice safer sex.


Chlamydia increases risk for HIV, leads to fertility and pregnancy problems, and might increase cancer risk.


The good news about chlamydia is that it’s easy to cure — but first, you need to know you have it! And that’s why it’s important for sexually active people to receive regular STD screening. Left untreated, chlamydia can increase risk of acquiring HIV, can hurt fertility in both males and females, can be harmful during pregnancy, and might even increase risk for a certain type of cancer. So why let it wreak havoc on your body when you could just get tested and take a quick round of antibiotics?

To find out just how seriously you should take chlamydia, let’s answer a few common questions about it.

Can Chlamydia Increase HIV Risk?

Chlamydia does not cause HIV. Chlamydia is caused by a type of bacteria, while HIV is a virus that causes a fatal disease called AIDS. However, many STDs, including chlamydia, can increase risk for an HIV infection, meaning that someone with an untreated chlamydia infection is more likely to be infected with HIV if exposed to the virus. STD Awareness: Will STDs Go Away on Their Own?

teensCan gonorrhea go away without treatment? Does chlamydia eventually clear up? Can trichomoniasis go away on its own? These are the kinds of questions people pose to Google before Google sends them here — at least that’s what I learned by looking at the blog’s stats. They’re tricky questions to tackle, and for so many reasons.

Some viral STDs stay with you for life, such as herpes and HIV. Others, such as hepatitis B and human papillomavirus (HPV), can be prevented with vaccines but cannot be cured. It’s also possible for the immune system to defeat hepatitis B virus and HPV — but in some cases, these viruses are able to settle in for the long haul, causing chronic infections that can endure for life and even lead to cancer.


Left untreated, syphilis can kill, and gonorrhea can cause infertility.


Non-viral STDs, like chlamydia and gonorrhea, can be cured. However, they usually don’t have symptoms, or symptoms can come and go, making it seem like an infection went away when it actually didn’t. You can’t know your STD status without getting tested, and you can’t self-diagnose an STD based on symptoms and then assume the infection went away when symptoms subside. Getting tested can uncover a problem and clear the way for treatment.

Nonetheless, people want to know if an STD can go away by itself — but there aren’t many studies on the “natural history” of curable STDs like gonorrhea, chlamydia, and trichomoniasis. Studying the natural course of a curable infection would require that scientists put their subjects at risk of the dangers of long-term infection, and no ethics board would approve such an experiment. STD Awareness: Is Bacterial Vaginosis a Sexually Transmitted Disease?

Bacterial vaginosis, or BV, is the most common vaginal infection among people 15 to 44 years of age. It’s caused by an overgrowth of harmful bacteria, such as Gardnerella vaginalis. A healthy vagina hosts thriving populations of Lactobacillus bacteria species, but when these “good” bacteria are crowded out by certain types of “bad” bacteria, the vaginal ecosystem can be shifted, causing BV.

There is a lot of confusion about BV. Is it a sexually transmitted disease (STD)? What are the symptoms? How can you avoid it?

All good questions. Let’s examine them one by one.

Is BV an STD?

The consensus seems to be that BV isn’t officially an STD, but even reliable sources have somewhat contradictory information. Planned Parenthood doesn’t list BV as an STD on their informational webpages. The Centers for Disease Control and Prevention (CDC) does include BV on their STD website, but also says that “BV is not considered an STD.”

On the other hand, the Office on Women’s Health says that “BV can … be caused by vaginal, oral, or anal sex” and that “you can get BV from male or female partners.” And there’s an entire chapter devoted to BV in the premier medical textbook on STDs, and its authors say that, while sexually inexperienced females can get BV, “the weight of evidence supports sexual transmission” of G. vaginalis, the bacteria species most famously implicated in BV infections.

The same webpage on which the CDC declared BV not to be an STD also says that it can be transferred between female sexual partners. Indeed, women who have sex with women have higher rates of BV. Since vaginal fluid could spread BV, partners can change condoms when a sex toy is passed from one to another, and use barriers like dental dams when engaging in cunnilingus (oral contact with the female genitalia) or rimming (oral contact with the anus).

Let’s Talk Contraception: Birth Control Pills — Not Just for Preventing Pregnancy

pillThere has been a lot of political posturing recently about whether the government should require health insurance to provide birth control without a co-pay as part of a preventive health care package. So many people, including politicians, can only “see” the contraceptive side, which is pretty important, by the way. Approximately 15.8 in 100,000 women in the United States die from pregnancy or pregnancy-related issues yearly, and that number has doubled in the past 25 years. We have one of the worst maternal death rates of all developed nations, right near the bottom of the list.


Birth control pills can be used to treat a variety of conditions, including painful periods, acne, endometriosis, and uterine fibroids.


But putting all that aside, let’s look at the how oral contraceptives pills (OCPs) are actually used in this country, and for what reasons besides contraception. You may argue that many birth control pills are only approved for contraception purposes by the Food and Drug Administration (FDA), so other uses are not valid. But many drugs that may have narrow conditions of approved use are often prescribed off-label by physicians when they have data and information about how effective they can be for other conditions where not much else works.

According to a 2011 study using data from the 2006–2008 National Survey for Family Growth, the Guttmacher Institute reported that 14 percent of all women using birth control pills — that’s 1.5 million women — use them for purposes other than preventing pregnancy. Granted, 86 percent of OCP users report using them for birth control. But over the years, these OCPs have helped many people as treatments for dysmenorrhea, menorrhagia, endometriosis, menstrual-related migraines, acne, uterine fibroids, and polycystic ovarian syndrome. In the microscopic world of germs, organisms called Chlamydiae are dwarfed by their fellow bacteria. An E. coli bacterium can hang out with 100,000 of its closest friends on the head of a pin, but Chlamydiae are smaller still. Infectious particles are about one-tenth the length of an E. coli, rivaling the size of a large virus. And, just like a virus, Chlamydiae can still pack quite a punch, proving that sometimes, not-so-good things can come in small packages.

There are many types of Chlamydiae bacteria, but one species, Chlamydia trachomatis, is responsible for not one, but two sexually transmitted diseases (STDs) in humans: chlamydia and lymphogranuloma venereum (LGV). (Humans aren’t the only ones affected by sexually transmitted Chlamydiae. A different species, Chlamydia pecorum, is devastating wild koalas in Australia, which has got to be one of the biggest bummers ever.)


Chlamydia is a case study for the importance of safer sex and regular STD testing.


Chlamydia is one of the most common STDs in the United States — there were almost 1.5 million diagnoses in 2011 alone, but experts estimate that there were around another 1.5 million cases of chlamydia that went undiagnosed. How can this be? Chlamydia is often a “silent” infection, meaning that symptoms are rare, allowing people to harbor these bacteria without even knowing it. (When symptoms do occur, they might include swelling in the genital region; vaginal, cervical, or penile discharge; or painful urination.)

It might seem like a small mercy that this common infection is unlikely to torture us with harrowing symptoms — but, in actuality, those of us who have to deal with discharge or burning urination should try to appreciate the heads up: Left untreated, chlamydia can cause serious complications. When it spreads along the female reproductive tract, it can cause pelvic inflammatory disease, which can severely compromise fertility and cause chronic pain. Rarely, in a male reproductive tract, it can cause epididymitis, which can also spell bad news for future fertility. Continue reading

STD Awareness: Hepatitis B Virus and the HBV Vaccine

July 28 is World Hepatitis Day. This month’s installment of our STD Awareness series will shine the spotlight on the hepatitis B virus (HBV), which can be transmitted sexually as well as nonsexually.

Hepatitis viruses infect the liver. Hepatitis A, B, and C can be transmitted sexually, and hepatitis B is the most likely to be spread this way. HBV is present in vaginal fluids, semen, and blood. It is highly contagious and can be transmitted by most sexual activities, such as vaginal or anal intercourse, as well as oral sex. HBV can also be spread by exposure to infected blood, and an HBV-infected mother can pass the virus onto her infant during birth.

To protect yourself from HBV, make sure to use latex barriers, such as condoms and dental dams, if you are sexually active. Also, don’t use unsterilized needles; don’t share hygiene items that could have infected blood on them, such as razors and toothbrushes; and consider being vaccinated against hepatitis B. Did you know that chlamydia (pronounced “kluh-MID-ee-uh”) is the most common sexually transmitted disease (STD) in the United States? However, many people with chlamydia may not even know they have it: 50% of men and 75% of women will have no symptoms. Sexually active individuals and individuals with multiple sex partners are at the most risk.

Like other STDs, many people with chlamydia are asymptomatic, some symptoms in men include:

  • Burning sensation during urination
  • Discharge from penis or rectum
  • Testicular tenderness
  • Rectal discharge or pain

The symptoms in women include:

  • Burning sensation during urination
  • Painful sexual intercourse
  • Rectal pain or discharge
  • Symptoms of Pelvic Inflammatory Disease, salpingitis, liver inflammation, similar to hepatitis
